Requirements to qualify for enrollment in our K–12 program
Only students who are five (5) years old before August 1st of the current school year and under twenty-two (22) years old are eligible to enroll.
Only students residing in Ohio are eligible to enroll. Provide one of the following as proof of residency:
Documentation must be current, include a street address, and be addressed to the student, parent, or custodial guardian.
We cannot accept:
Residency documentation should reflect a good faith effort to accurately identify the student’s residence. Additional methods apply for students who meet the McKinney-Vento definition of homeless.
Students are considered homeless if they lack a fixed, regular, adequate nighttime residence or are doubled-up due to loss of housing, economic hardship, or similar reasons, including living in motels, shelters, or hospitals.

The following immunizations are required by the first day of school:
Waivers: May be submitted under Ohio Statute 3313.671 or other applicable law.
Custody Documentation: Documentation must be stamped court copies.
Internet Service: Families are responsible for obtaining reliable internet access. Internet reimbursement is available upon request.
Submitting an application does not guarantee enrollment. Students should not be withdrawn from their current school. Once a student attends a new student orientation and begins working in their coursework QDA will send a notice of enrollment.
