Enrollment Eligibility

Requirements to qualify for enrollment in our K–12 program


Age Limitations

Only students who are five (5) years old before August 1st of the current school year and under twenty-two (22) years old are eligible to enroll.

Ohio Residency

Only students residing in Ohio are eligible to enroll. Provide one of the following as proof of residency:

  • Utility Bill (Gas, Electric, or Water)
  • Deed, mortgage, lease, homeowner’s or renter’s insurance declaration page, or current real property tax bill
  • Paycheck or paystub issued within 90 days including primary residence address
  • Current bank statement including primary residence address
  • Documented affirmation of address from district of residence
  • Notarized affirmation from parent/student if over 18
  • USPS return receipt from certified letter sent by district
  • Written confirmation from Department of Job and Family Services
  • Written confirmation from local law enforcement agency

Documentation must be current, include a street address, and be addressed to the student, parent, or custodial guardian.

We cannot accept:

  • Driver's License or Passport
  • Voter Registration
  • Cellphone or cable bill

Residency documentation should reflect a good faith effort to accurately identify the student’s residence. Additional methods apply for students who meet the McKinney-Vento definition of homeless.

Students are considered homeless if they lack a fixed, regular, adequate nighttime residence or are doubled-up due to loss of housing, economic hardship, or similar reasons, including living in motels, shelters, or hospitals.

Kindergarten Immunization Records

The following immunizations are required by the first day of school:

  • 4 doses of DTaP (5th dose if all before age 4)
  • 3 doses of Polio (last after age 4)
  • 3 doses of Hepatitis B
  • 2 doses of MMR (Measles, Mumps, Rubella)
  • 2 doses of Varicella (Chickenpox)

Waivers: May be submitted under Ohio Statute 3313.671 or other applicable law.

Other Required Items

Custody Documentation: Documentation must be stamped court copies.

Internet Service: Families are responsible for obtaining reliable internet access. Internet reimbursement is available upon request.

Important Enrollment Notice

Submitting an application does not guarantee enrollment. Students should not be withdrawn from their current school. Once a student attends a new student orientation and begins working in their coursework QDA will send a notice of enrollment.
